20 DAY AGREEMENT

This agreement provides your department with one Blue Racer Headset for testing and evaluation purposes. Due to the high demand for demonstration units, we must ask that the headset be returned promptly per the conditions of this agreement. Failure to return the unit within the specified terms will result in an invoice being issued to your department with a copy of this agreement attached.

Please fill in the form below completely and click submit when finished. Please be sure to provide a telephone number and time when you can be reached by our offices to further discuss your needs prior to a T&E unit being forwarded.

The terms of this agreement are: (a) You agree to complete an evaluation of the product on department letterhead including demo performed by, situation(s) used in, comments on performance, utility, comfort, comparison to other headsets and suggestions (b) You agree to return the product within the 20 day evaluation period.

The information you provide on this electronic document will be used only for contacting you and providing your department with a T&E headset. Your information will never be shared with any other company or individual not associated with Atlantic Signal, LLC.
